This lecture will present the development of guided bone regeneration in implant dentistry since the late 1980’s. Different surgical techniques and use of autogenous bone graft, bone substitutes and barrier membranes to optimize the treatment outcome and reduce post-surgical complications will be presented and illustrated by clinical and radiographic cases with a 10-years observation period. A special emphasis will be given to esthetic sites in the anterior maxilla and to implant sites in the posterior mandible.

Regeneration of bone in conjunction with implant placement is an inseparable part of modern implant therapy. Whereas about 40% of implant sites throughout the oral cavity are performed with some form of bone augmentation, this value mounts to almost 100% in esthetic areas. On the one hand, adequate bone integration of the implant is necessary to absorb the loading forces and on the other hand the bone is also critical for achieving and maintaining stable and pleasing soft tissue esthetics. This presentation will discuss different clinical procedures and materials to augment bone in esthetic implant sites and how this bone reacts over time.

Soft tissue grafting procedures are increasingly performed in conjunction with dental implant therapy to improve the peri-implant soft tissue health and esthetics. In this presentation, three important indications and different surgical techniques to enhance the amount of peri-implant keratinized tissue, increase the thickness of peri-implant tissues and treatment of peri-implant soft tissue dehiscence will be presented. Furthermore, indications, surgical techniques and used biomaterials will be discussed.

Managing peri-implantitis is currently one of the biggest unsolved problems in implantology and its prevalence is increasing. Due to the macro- and microstructure of dental implants, the effective decontamination of implants is difficult with conventional methods like brushes, chemicals, or lasers. In this lecture, treatment of peri-implantitis and limitations will be discussed as well as GalvoSurge in the treatment of peri-implantitis.
